LA PRODUCCIÓN DE SALUD

Decimos “producción de salud” para descentrarnos de conceptos que consideran la salud sólo desde los procesos mórbidos sin considerar los mecanismos psíquicos que intervienen en el sujeto.

¿Qué conceptos de salud conocemos?

La Salud ha sido definida por la OMS como “el estado de completo bienestar físico, mental, social y no solamente la ausencia de enfermedades”. Siendo la enfermedad definida como la “alteración o desviación del estado fisiológico en toda la economía o en alguna de sus partes, órganos o sistemas que se manifiesta por un conjunto característico de síntomas y signos, cuya etiología, patología y pronóstico pueden conocerse o ser desconocidos.

Si hablamos de salud psíquica, la psiquiatría la delimita como “la normalidad”, es decir se piensa la normalidad como salud y se parte de criterios estadísticos donde lo normal es lo que aparece con mayor frecuencia. Esta concepción presenta claras deficiencias pues no discrimina las “anormalidades” que por exceso o defecto se sitúan en ambos extremos en la curva de Gauss, por ejemplo los superdotados no son normales. Este enfoque de normalidad se vincula con criterios sociológicos en los que la idea de salud se relaciona con la adaptación social y con la reacción positiva que el individuo provoca en el grupo social.

Otras nociones psicológicas piensan la salud psíquica como “el equilibrio armónico” entre los diferentes elementos del aparato psíquico (Ello, Yo y Superyó). Estas teorías parten del concepto de psicopatología, es decir, del estudio de las dolencias del alma, definiéndose como la ciencia que toma su objeto de la psiquiatría y su espíritu de la psicología. En definitiva la psicopatología es la fundamentación científica de la psiquiatría, describiendo las alteraciones y perturbaciones de la conciencia, de la atención y orientación témporo-espacial, de la memoria, de la percepción, de la afectividad… la psicopatología es una ciencia de causa-efecto.

El psicoanálisis es una ciencia de efectos, en su faceta terapéutica no trata las dolencias del alma, sino desde el alma y descentrándose de la razón, lo aparente o lo visible, permite la producción de salud aunque el sujeto padezca alguna enfermedad. Sabemos que lo psíquico no se adapta sino que se transforma, insiste, se repite, siendo imposible la concordancia o adaptación entre la palabra y la cosa, entre lo que demando y lo que deseo, lo que busco y lo que encuentro, lo que pienso y lo que soy. No hay posibilidad de armonía entre las instancias psíquicas (Yo, Ello y Superyó) sino un equilibrio inestable que no cesa de relacionar el sistema inconsciente con el sistema preconsciente-conciencia, estando dichas instancias psíquicas sobredeterminadas y múltiplemente determinadas a nivel metapsicológico: económico, tópico y dinámico.

We say "health output" to distract ourselves from concepts that consider health only from morbid processes without considering the psychic mechanisms that intervene in the subject.

¿Qué conceptos de salud conocemos?

Health has been defined by the WHO as "the state of complete physical, mental and social well-being and not only the absence of diseases." Being defined disease as the “alteration or deviation of the physiological state in the entire economy or in any of its parts, organs or systems that is manifested by a characteristic set of symptoms and signs, whose ethology, pathology and prognosis may be known or unknown.

If we talk about mental health, psychiatry defines it as "normality", that is, normality is thought as health and it is based on statistical criteria where normal is what appears most frequently. This conception has clear deficiencies since it does not discriminate the "abnormalities" that by excess or defect are located at both ends of the Gaussian curve, for example, the gifted are not normal. This approach to normality is linked with sociological criteria in which the idea of health is related to social adaptation and to the positive reaction that the individual provokes in the social group.

Other psychological notions think of psychic health as "the harmonic balance" among the different elements of the psychic apparatus (Id, ego and Super-ego). These theories start from the concept of psychopathology, that is, from the study of the ailments of the soul, defined as the science that takes its object from psychiatry and its spirit from psychology. All in all, psychopathology is the scientific grounds of psychiatry, describing the alterations and disturbances of consciousness, attention and temporal-spatial orientation, memory, perception, affectivity ... psychopathology is a science of cause- effect.

Psychoanalysis is a science of effects, in its therapeutic facet it does not treat the ailments of the soul, but from the soul and by deviating from reason, the apparent or the visible allows the production of health even if the individual suffers from some disease. We know that the psychic does not get adapted instead it is transformed, it insists and repeats itself, concordance or adaptation between the word and the thing being impossible, between what I demand and what I want, what I seek and what I find, what I think and what I am. There is no possibility of harmony between psychic instances (I, It and Super-ego) but rather an unstable equilibrium that does not cease to relate the unconscious system with the preconscious-consciousness system, being these psychic instances overdetermined and multiply determined to the metapsychological level: economic, topical and dynamic.
